精准探访暖人心 科学评定护安康——衡阳市雁峰区全面开展特困供养对象探访与护理等级评定工作

为切实筑牢民生保障底线，精准掌握分散特困供养对象生活状况与照护需求，近日来，衡阳市雁峰区民政局联合区社工站，深入开展特困供养对象入户探访与护理等级评定工作，让特殊群体感受到“政策温度”与“专业守护”。

此次工作以“精准评估、不落一人”为原则，组建了由民政工作人员、街道专干、专业评估员构成的工作小组，通过“一看、二问、三评估”的方式推进工作。“一看”即实地查看特困对象居住环境、饮食起居情况，排查安全隐患；“二问”即与对象本人及护理人员深入交流，详细了解其身体状况、生活困难及服务需求；“三评估”则由专业评估员依据《特困人员认定办法》及护理等级评定标准，从自理能力、基础运动能力、精神状态、感知觉与社会参与能力等维度进行科学打分，精准划分护理等级。

截至目前，全区已完成31名分散特困供养对象的探访与评定工作，后续将根据评定结果，及时更新护理等级，为特困对象精准匹配居家护理、机构照料等服务，并建立动态跟踪机制，定期更新对象需求与护理等级，确保民生保障政策“精准滴灌”。区民政局相关负责人表示，下一步将持续优化特困供养服务体系，推动探访关爱与护理保障常态化、专业化，切实保障好特困群体的基本生活。（ 周俊杰）
